Certified Phlebotomy Instructor Certification Renewal/Reinstatement
- $100 Renewal Fee
- + $50 late fee for renewals paid 30 days past expiration date
- + $115 fee assessed for renewals paid 90 days past expiration date
If you are 30 or 90 or more days past your expiration date, you MUST pay the applicable fee to renew your certification. Please type your certification number in the Certification Number field and select the reinstate fee.
It is up to the Instructor to maintain their certification and pay the renewal on or prior to the renewal date.If the incorrect Renewal Amount is paid and a balance remains you are not compliant. When your renewal is processed, unless there was a documented error on our part, there are no refunds submitted. Please allow for 6-8 weeks for processing.
